Over-Assessed and Under-Treated: Why Some Patients Never Return After Their First Physiotherapy Session

Have you ever been to a physiotherapist, osteopath, chiropractor, myotherapist, or other health professional and felt like you were assessed for the whole session, but barely treated?

You walked in with pain, stiffness, or an injury hoping for help.

Instead, you spent most of the appointment answering questions, doing movement tests, being measured, and listening to long explanations.


Assessment is important. Clinical reasoning matters. Understanding the cause of pain matters.


But when assessment takes over the entire session and treatment is left until the final few minutes, many patients leave feeling disappointed.


They may understand their condition slightly better, but they still feel sore, restricted, uncertain, and unsure whether the appointment actually helped.


And often, they never return.


Why Over-Assessment Can Be a Problem

In physiotherapy, assessment helps identify what may be contributing to pain or injury. It allows the clinician to understand your symptoms, goals, movement, strength, mobility, and functional limitations.

  • A plan for recovery, sport, work, or daily activities

A good first physiotherapy consultation should provide both clarity and action.


Assessment Matters, But Treatment Matters Too

This does not mean making unrealistic promises or rushing the assessment.

It means helping the patient experience progress early.


For example, a patient with back pain may leave moving more freely. A runner with knee pain may understand how to modify training safely. Someone with neck pain may feel less restricted after treatment and know what to do at home.


Small improvements can build trust.


They show the patient that their problem is understood and that there is a practical pathway forward.
